Carolina Outreach is seeking a Forensic Assertive Community Treatment (FACT) Registered Nurse to deliver medical and psychiatric care to individuals with serious mental illness and co-occurring substance use disorders who are involved in the criminal justice system.

As a Forensic ACT RN you will provide medication management health monitoring crisis intervention and care coordination while working closely with law enforcement courts probation/parole and correctional healthcare providers to ensure continuity of care and successful community reintegration. This role emphasizes forensic treatment planning risk assessment and legal system collaboration to support justice-involved individuals.

Position Highlights:

  • Administer and monitor medications provide health education and ensure adherence to prescribed treatments.

  • Conduct health assessments and screenings to identify physical and psychiatric needs requiring medical or clinical intervention.

  • Collaborate with criminal justice and behavioral health partners to coordinate medical and psychiatric care for justice-involved individuals.

  • Support reentry and continuity of care for individuals transitioning from incarceration to community-based services.

  • Participate in multidisciplinary team meetings providing medical input crisis support and documentation in accordance with regulatory standards.

Qualifications

  • Education:
    • Graduation from an accredited nursing program.
  • Certification / Licensure Requirements:
    • Must be a Registered Nurse (RN) currently licensed to practice in the state of North Carolina.
    • Certifications: Crisis Prevention Intervention (CPI) certification required within 90 days of hire. CPR/First Aid certification required.
    • Valid Drivers License and Reliable Transportation required.
  • Experience / Knowledge Requirements:
    • Preferred: Experience in crisis intervention emergency psychiatry or correctional nursing.
    • Knowledge of criminal justice system processes forensic risk assessment and reentry planning.
    • Experience working with justice-involved individuals with mental illness and co-occurring disorders.
